Please let our scheduler know where you want to be seen - We will accommodate you with the earliest appointment convenient to you! Remember to bring the CD with any MRI, X-ray or CT scan studies you have had done previously. DO NOT ask your doctor or radiology center to mail or Fed Ex studies to my office. They never arrive in time! Bring the reports as well, and any reports of previous physical therapy or pain management. |

If you are being seen for a workers compensation injury, or are a member of an HMO, we are glad to take care of you, but you will need to get a referral and approval from your Primary Medical Doctor or Physician of Record, before you can be seen.
Please bring all pertinent x-rays, MRI, or CT studies on a CD disk. Bring the reports as well, and any reports of previous physical therapy or pain management.
Bring a list of your current medications and supplements, and your current insurance card and driver's license.
Unless you have recently had x-rays, and have them with you on a CD, we will want to get good quality films of the area of concern. Please come 30 minutes prior to your appointment in order to get these studies done before you are scheduled to be seen.
